How to Participate in the NFPTR
To join the registry, we ask that you or your family complete a brief questionnaire regarding your family's cancer history.
After your request is received, we will send you a link to our secure online questionnaire.
If you prefer to complete a paper version, please let us know and we will send you a paper copy via mail. Also, in selected cases, we may contact you to request the donation of a DNA sample for our research study. Click here to learn why we request blood or other DNA samples. We may also contact you if we have additional questions.
The NFPTR is protected by a Certificate of Confidentiality (NCI-01-062) from the National Institutes of Health, Department of Health and Human Services. This certificate helps us to protect the information that you provide to our registry.
If you do not qualify for or do not wish to join the registry and still would like to help our research, you can donate to the NFPTR.
